Mathnasium  located on First St in Pleasanton, is offering a free diagnostic and 1 month of tutoring!  They also have locations in San Ramon, Union City, Walnut Creek, Alameda, Oakland, and a few other places as well.  Their program is for children from pre-K through 12th grade.  Please go to their website for an overview of their program and method.
